Initial Submission Forms
If applicable to your study, the completed forms should be sent to HREBA for review.
- HREBA Billing Information Form (Industry funded studies only):
Upload the completed Billing Information Form to the “REB Service(s) Fee” section. Once the initial ethics certificate has been issued an invoice will be sent to the e-mail indicated on the form.

Study Renewal and Completion Forms
Use the forms below for a study that is currently not in IRISS. The completed forms and accompanying documents should be mailed to HREBA for review. (address below)
If your study is in IRISS, complete your renewals or requests in the system; do not send paper documents to HREBA.

Use the forms below to make modifications to a study that is currently not in IRISS. The completed forms & accompanying documents should be mailed to HREBA for review. (address below)
