- TCPS2 tutorial completion certificate
- Current health professional practice permit

If you are already registered with one of the HREBA committees and need access to another committee, please contact us. Submitting another registration will not give you access to the additional committee.
Please Note: Minimal Risk studies such as Phase 4 Observational Post-Marketing studies or Chart Reviews should be submitted to HREBA – Community Health Community (HREBA-CHC). If you are unsure where to apply please contact info@hreba.ca.
