Initial Submission Forms

If applicable to your study, the completed forms should be uploaded to IRISS for review.

• HREBA Billing Information Form (Industry funded studies only):
  Upload the completed Billing Information Form to the “REB Service(s) Fee” section.  Once the initial ethics certificate has been issued an invoice will be sent to the e-mail indicated on the form.
• HREBA Departmental or Site Approval Form
• Board of Record Agreement for investigators conducting research through private practice or other community-based organizations (both for and not-for profit)

Study Renewal Form

All renewals are to be completed within IRISS. If your IRISS file is associated with multiple sites in Alberta, the participating site should complete the following form and upload to the renewal file in IRISS by clicking “Log Comment to REB Admin”.

• Renewal for Participating Sites – IRISS Multisite Studies 

Site Completion Form

If your IRISS file is associated with multiple sites in Alberta and only one of the sites is closing, the closing site should complete the following form and upload to the modification file in IRISS (Section 11.0 – Other Documents, of the Documentation Page).
